To ensure your seamless participation in the event, please download and complete all the intake paperwork and packets provided below. These documents are crucial for your attendance and advancement through the program. We look forward to seeing you at the workshop and guiding you on your journey to homeownership!

Please be advised that paperwork packets cannot be submitted on the day of the event. Participants are encouraged to email their completed paperwork and supporting documentation to [email protected] ahead of time. If more than one participant, page 1 and page 4 are individuals, the rest of the package has spaces for two participants.

Empowering Homeownership: The Critical Next Steps

Complete the following steps before the event. Submit your paperwork and documents before March 20, 2024.

Download Intake Packet

Complete the intake form. We have both English and Spanish forms available.

Create Copies

Produce digital copies of the following:

  • Copy of your driver's license or photo ID
  • 2 years of signed tax returns
  • 3 months of bank statements
  • Your most recent paystubs reflecting one (1) month's salary
  • If you receive alternative income (i.e. child support, Social Security, Pension, etc.) , please provide an award letter
  • If self-employed, a copy of Schedule C from your most recent tax return

At the end of the workshop, you will be invited to a one-on-one consultation with our HUD Certified Homeownership Experts! This HUD-Approved workshop (part A) qualifies you for Down Payment Assistance and Subsidy Programs offered throughout Florida! You will receive a Homebuyer's Certificate once you complete the process.
